For Physical Therapy

Registration paperwork must be completed prior to the initial physical therapy session. You can complete the paperwork from your patient portal after scheduling your physical therapy evaluation. If you choose to complete the paperwork at the time of your session, please arrive 15 minutes early so that all documentation can be completed prior to your start time.

For Group Classes

A consent form must be completed prior to participating in a group class. You can fill out and submit this brief form electronically from your account. You can use the link below to sign up for a new account or sign into your current account. If you choose to complete the form at the time of class, please arrive 10 minutes early so that everything can be completed before class begins.
